Dementia passport

The Dementia Passport is a collaborative project between CMHT, Alzheimer’s Scotland and UCI.

Personal Info is recorded on the Dementia Passport by the adult themselves or by their carer. A photograph is also on the Dementia Passport.

When a person with memory problems / dementia is admitted to hospital, it is envisaged that the Dementia Passport will help the hospital help the person. The Dementia Passport is not a medical document.
